REQUESTING MEDICAL RECORDS

Your medical records are always free and accessible through our secure online patient portal.

To maintain patient confidentiality and ensure compliance with privacy laws:
There Is A Fee
You will be charged for copies of medical records requested if you are a non-contracted agency that is not the direct payer of services. You will be charged if you are not private council.
Written Requests Only
We do NOT respond to phone or fax requests for medical records.
We do not release medical records without a formal written request.
No Phone Verifications
We do not verify patient information over the phone, even when speaking with insurance companies or attorneys.
We do not fax medical records to insurance companies or attorneys.
Secure Delivery Required

Fees for Medical Records:
Current fees (fees are subject to change without notice):
Electronic records (secure e-mail documents): $25
Electric fax $25 plus $2 for each note
Paper records (printed and mailed): $35 plus $5 for each note
