My PT Hub is an app that you can access online or via your phone. We use My PT Hub to create and share your custom workout and track your progress. You use the app at home to help you do the exercises we’ve provided correctly.

Good Faith Estimate

Under the law, health care providers like You’re In CONTROL! need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided. When you are in touch with us about beginning therapy, we’ll check your insurance and provide you with the necessary information about expected out of pocket costs.

Request Access To Our Patient Portal (HelloNote)

Below you’ll find information about how to request access to our client portal (provided by HelloNote).

Step One:

Request access to our Patient Portal by filling out the form below.

    Next Steps:

    After you submit your portal request and we have entered your information into our computer and scheduled your evaluation, you will receive two emails from HelloNote. 

    You must respond to these two emails from HelloNote within 24 hours of receiving them.

    The first email you receive is simply an attempt to confirm your email address. You will need to click a link in this email. 

    The second email you receive provides your username and a temporary password, which you will use to access the patient portal for the first time. Please write down your username and temporary password. Once you follow the link in that email it will take you directly to HelloNote’s login page.

    (If you can’t find the emails, check your spam and/or updates folders, or search “HelloNote” in your mail app.)

    If you do not respond in time to the confirmation or login credential emails, you can use this link https://app.hellonote.com/UserAccount/RequestResend to request access.

    Please look at this information and make sure everything is correct. If it is not, please let us know ASAP by using our general contact form here. Also, let us know about any changes to your insurance right away.

    Appointments Tab

    Here you can view, cancel, or request a change to your upcoming appointments. Please try to send any cancellations or change requests at least 48 hours before your scheduled appointment. Your requests will be answered by our administrative staff in a timely manner. 

    Intake Forms Tab

    As a part of our new patient onboarding process we’ll ask you to fill out these forms and press submit to upload them to your chart PRIOR to your treatment. Your therapist will review this paperwork prior to your appointment. This way the focus is on you at your visit, not your paperwork! 

    Sometimes we send new clients a request to complete specific forms prior to these clients having access to the portal. If you already completed forms and submitted them to us, you don’t need to complete the same forms when you log into the portal. You can ignore this tab.

    Payments and Visits Tab 

    You can view your previous visits, payments you’ve made, and any balance due. This tab is where you make payments to your account.

    Documents Tab

    You can view the forms you filled out and any other documents that have been uploaded to your chart. Upon request, your medical records can be uploaded to this tab by our office staff for your review.

    Other Tabs

    We do not currently use the other tabs on this page.
